A blurry spot, a wavy line, or even a darkness you barely notice at first can sometimes point to changes deep inside your eye. If you notice any changes like these, it may be time for an optical coherence tomography (OCT) scan.
An OCT scan gives your eye doctor a close-up view of your macula. The macula is the part of your retina that helps you see things directly in front of you and process details like faces, words, small movements, and colors.
This quick, noninvasive scan can help your doctor look for early signs of age-related macular degeneration (AMD), check whether treatment is working, and watch for subtle changes in your eyes.
Optical coherence tomography is an imaging test that uses light waves to take detailed pictures of the back of your eye. It creates cross-sectional images, which means your doctor can look at the retina in layers instead of only seeing its surface.
An OCT scan allows for detailed imaging of the macula, the area of the eye affected by AMD. The macula allows you to do things requiring focus on details, like reading, driving, and recognizing faces.
OCT is an important tool for diagnosing AMD, but it’s not the only one. Doctors usually make an AMD diagnosis by combining a clinical eye exam with imaging.
OCT can be especially helpful because early AMD may not cause symptoms. You may not notice early warning signs of vision loss until the condition has already affected your macula. With OCT, your doctor can look for specific findings that point to AMD.
OCT can help doctors tell the difference between dry AMD and wet AMD, although other tests may still be needed. It’s possible to have both dry AMD and wet AMD in the same eye.
Dry AMD is more common and generally progresses slowly. It often involves drusen and thinning of the macula. Wet AMD is generally more severe and involves abnormal blood vessels that can leak fluid or blood under or into the retina. The growth of these blood vessels is called choroidal neovascularization (CNV).
Some eye specialists also use OCT angiography (OCTA). OCTA is a related imaging test that can show blood vessel anatomy in the retina and choroid, a blood vessel layer under the retina. OCTA can help doctors view abnormal blood vessels in wet AMD.
After an AMD diagnosis, OCT can help your doctor monitor progression — whether the condition is stable (staying the same), worsening, or otherwise changing. Because OCT scans show the retina in layers, your doctor can compare your latest scan with earlier scans to look for changes over time.
This comparison may show whether drusen have changed, the macula is thinning, or new fluid has appeared. Such changes in your eye health can help your doctor decide on the best treatment and how often you need follow-up visits.
Monitoring is also important because AMD can be different in each eye. One eye may have early or intermediate dry AMD, while the other eye may have more advanced changes. It’s also possible for AMD to be present in only one eye.
Regular OCT scans may help your eye doctor catch important changes sooner, sometimes before you notice a difference in your sight.
Treatment for AMD will often depend on whether it’s dry or wet AMD, and how advanced it is. For wet AMD, doctors often use OCT to see whether anti-vascular endothelial growth factor (VEGF) treatment is helping.
Anti-VEGF medicines are injected into the eye to help control abnormal blood vessel growth in CNV. Your doctor may use OCT to check whether leakage is improving after treatment.
OCT results can also help guide timing. Some people need injections on a regular schedule. Others may adjust the time between treatments based on how their eyes look and how their vision is doing.

For geographic atrophy, an advanced form of dry AMD, imaging tests can help doctors measure areas of atrophy and watch how they change. Catching geographic atrophy early and starting treatment right away can help slow its progression.
OCT scans are useful, but they aren’t perfect. Your doctor will still consider your symptoms, vision tests, clinical exams, and sometimes other images when making treatment decisions.
It’s natural to wonder how to read OCT scan results. The black-and-white lines, curves, and shadows can look confusing. Your doctor is trained to read these images and connect them with your symptoms, vision test, and eye exam.
